Staff are responsive, professional, and extremely kind to all of my needs.  Food is decent.  I would highly recommend this facility for any after hospital stay. Brook at High Falls is a state of the art facility, anyone that needs good care and a beautiful place to come for rehab it's definitely the #1 place to be. I am not going to name names as I'm afraid I'll forget someone.  The entire staff from bottom to top was more than wonderful and did far more than was expected. I can't say enough good things about his stay with the wonderful staff at the Brook. Obviously no one wants their loved one to have to live in a nursing home but circumstances usually leave our loved ones no other choice. If this happens to your family you can confidently allow this loving staff to care for them from Rehab to long term care. My dad continued to tell us that the food is excellent and he was an Italian- picky eater. The daily staff kept him clean and made him laugh with their teasing and smiles. The Activities staff Kristina and Alice were so accommodating in the COVID rules and restrictions. We could always count on them to keep in constant contact. Nursing staff really made us feel that my dad and his well being were top priority. PT and OT staff did their best to keep my dad as physically fit as possible. The facilities are a bit run down and they could use some upgrades (new beds) but the new owners have definitely put their resources in the right place with the staff. Thank you everyone at the Brook for giving my dad a home for 9 months.

First review from June 2020: My 91 year old dad has been there a few weeks first in rehab and now in long term. I was really concerned with the COVID rules but the staff has gone above and beyond accommodating window visits and zoom meetings. Kristina, the Activities Director, has been especially helpful. All staff I have encountered are genuinely concerned for my dad. He also reports the “food is good”. I can’t say that for the other 2 facilities he has had to go to for rehabs after a fall. They have had some trouble with their new phone system so I have experienced issues getting a live person on the weekends but they tell me they are working on it. Thanks so much Brook at High Falls for offering affordable, great care!

24-Hour Nursing & Supervision Care: Our dedicated staff provides round-the-clock nursing and supervision, ensuring that all medical needs are met promptly and efficiently, day or night.
Well-Balanced Meals: Residents enjoy three well-balanced meals daily at 9 AM, 1 PM, and 6 PM. Meals can be enjoyed in the communal dining room or delivered directly to residents’ rooms for a more private dining experience.
On-Site Therapy Team: Our professional therapy team includes specialists in physical, occupational, and speech therapy. They offer customized treatment plans to support each resident’s unique health goals.

Admissions

When faced with the decision of selecting a nursing home in Rochester, NY, it can feel overwhelming. At The Brook at High Falls Nursing Home, we understand this and have developed a user-friendly admissions process that includes an on-site Social Worker and simplified forms. Rest assured, we accept Medicaid, Medicare, long-term care insurance, and private insurance.
Admission steps to complete are different for those entering The Brook from home or the hospital. Coming from home, the process may take up to a week. The completed documents needed include:

Patient Review Instrument (PRI) (completed by a certified professional)
Screen (completed by a certified professional)
Admissions Application

If your loved one is currently hospitalized, kindly inform your social worker about your interest in our independently operated nursing home. Request a referral to be sent to The Brook, and rest assured that the necessary information will be handled by the social worker.

The Director of Social Work/Admissions Coordinator at The Brook will carefully review the provided information alongside the Director of Nursing and Administrator. A decision will be reached within 24-48 hours. Once the paperwork is submitted, we highly encourage family members and friends to schedule a tour of The Brook, allowing them to experience firsthand the warm and caring environment we offer.

Upon admission, new residents at The Brook at High Falls Nursing Home are given the option to choose between a spacious private room or having a companion roommate.
