Admissions

With the assistance of an on-site Social Worker and streamlined forms, we have created a user-friendly experience. Rest assured, we accept Medicaid, Medicare, long-term care insurance, and private insurance.
The admission process for individuals entering Wedgewood differs depending on whether they are coming from home or the hospital. If coming from home, the process may take up to a week to complete. The required documents include the following:
- Patient Review Instrument (PRI) (completed by a certified professional)
- Screen (completed by a certified professional)
- Admissions Application
If your loved one is currently hospitalized, please inform the social worker about your interest in our independently operated nursing home. They will assist in sending the necessary referral to Wedgewood.
The Director of Social Work/Admissions Coordinator at Wedgewood will carefully review the information alongside the Director of Nursing and Administrator. A decision will be reached within 24-48 hours. Once the paperwork is submitted, we encourage family members and friends to schedule a tour of Wedgewood and discover all that we have to offer.
Upon admission, new residents at Wedgewood Nursing Home are given the choice between a spacious private room or a companion roommate.
